All applicants have a continuing duty to report immediately, in writing, any change in your personal, educational or professional status that may affect your standing relative to your character and fitness to practice law, including anything that would modify any of the responses on your Application, Character & Fitness Questionnaire or subsequent updates.

*This form is only to amend/update a current application on file (no fee required). Do not use this form for address changes. Use the Change of Address form.
